How do I make an appointment? |
|
To request an appointment with one of our physicians or to schedule tests in the office or hospital, please call our main number (305) 279-4500 and follow the telephone prompts for your physician. If you are a new patient to the practice, you may dial prompt No. 2 for immediate access and information.
Our physicians see patients by appointment from 9:00am until 4:30pm, Monday through Friday. While you may request a specific physician when referred to the practice, you may be seen by other physicians or physician assistants over the course of your treatment. This approach helps team the patient with subspecialists to ensure familiarity with needs specific to each patient's case.
If you feel that you are experiencing a medical urgency, please do not hesitate to call our office, regardless of the day or hour. We have in place a system of physician coverage that ensures that urgent medical needs are met promptly.
When requesting an appointment, the scheduler may ask the following information:
> Your name
> Your date of birth
> Home or alternate phone numbers (work or cell)
> Your primary or referring physician
> Your insurance company or HMO information

Yes. To expedite your visit to our practice, we encourage our patients to access the ON-LINE FORMS page of our website for a listing of forms to complete prior to their visit.

Do I need a referral to be seen by a cardiologist? |
|
If you participate in an HMO or Point-of Service (POS) health plan, we request that patients obtain a referral or authorization from their primary care physician prior to their appointment and to bring this referral form to their visit.

Is there free parking at your facility? |
|
There is free parking in the facility. Further, we have valet service at the lobby entrance for a fee of $3 in case you cannot find satisfactory parking.

In what hospitals do the physicians see patients? |
|
The physicians have privileges at both Baptist Hospital and South Miami Hospital.
|

How do I obtain prescription refills? |
|
We will renew medications prescribed by our physicians. In order to keep your prescriptions current, we suggest that you visit our office at recommended intervals. The easiest method of having your prescription renewed is to have your pharmacist fax your request to our office at (305) 598-1741 and our medical assistants will take care of your prescription refill within 24-48 hours.
